Pulmonology

Smudra-Sinusitis, Migraine, and Untreated Diseases Research Association; we consider it a privilege to serve you. Smudra is the first Ayurvedic and naturopathic institute in India to discover a treatment for sinusitis and migraine using Indian herbs—and has demonstrated that this is possible with just a 1-day of treatment.

| Gender: | Male |
| Age: | 46 Year Old |
| Acquie Diseases: | Asthma |
Chief Complaint: Persistent breathlessness, frequent nocturnal awakenings, and recurrent exacerbations despite maximal conventional therapy for over 8 years.
The patient was diagnosed with bronchial asthma at age 32. Over the years, symptoms progressed from intermittent to severe persistent asthma. She experienced:
She also reported longstanding allergic rhinitis, post-nasal drip, and occasional facial pressure — symptoms that often overlapped with her asthma flares.
Final Diagnosis: Severe eosinophilic (Type-2 high) asthma with comorbid allergic rhinitis and chronic rhinosinusitis — classified as difficult-to-treat progressing to true severe/refractory asthma.
According to GINA guidelines, severe asthma is defined as asthma that remains uncontrolled despite optimized high-dose inhaled corticosteroid (ICS) + long-acting β-agonist (LABA) therapy and management of contributory factors, or that requires high-dose treatment to remain controlled.
Approximately 5–10% of asthma patients fall into this category. They account for a disproportionate share of healthcare costs, hospitalizations, oral steroid burden, and reduced quality of life. Key features include:
Stepwise escalation followed GINA recommendations:
| Stage | Treatment | Response |
|---|---|---|
| Initial | Medium-dose ICS/LABA + montelukast | Partial control |
| Escalation | High-dose ICS/LABA + LAMA (triple therapy) | Improved lung function modestly but exacerbations continued |
| Add-on | Theophylline, azithromycin trials | Minimal benefit |
| Biologic 1 | Omalizumab (anti-IgE) for 12 months | Reduced some allergic symptoms; exacerbations persisted |
| Biologic 2 | Mepolizumab (anti-IL-5) | Eosinophils dropped; still required intermittent OCS |
| Consideration | Benralizumab, Dupilumab, or Tezepelumab | Discussed as next options for refractory eosinophilic asthma |
Despite these measures, the patient continued to experience breakthrough symptoms and could not fully discontinue oral steroids without relapse.
Given the strong upper-airway comorbidity and the patient’s interest in reducing long-term steroid burden, a multidisciplinary plan incorporating Ayurvedic principles was introduced alongside optimized conventional care (in the spirit of clinics specializing in sinusitis–migraine–allergic respiratory conditions such as SMUDRA).
Key elements included:
This integrative strategy aimed to reduce the inflammatory load from the upper airways, improve overall respiratory resilience, and support the action of biologic or conventional therapies.
After 6–9 months of optimized conventional therapy plus targeted upper-airway and supportive Ayurvedic management:
The case illustrates that even in severe resistant asthma, addressing treatable traits (especially comorbid sinus and allergic disease) and using a precision + supportive approach can meaningfully improve control and quality of life.
Severe asthma resistant to standard therapy remains a significant clinical challenge. Success requires systematic assessment, phenotype-driven advanced treatments (especially biologics), aggressive management of comorbidities, and, where appropriate, complementary modalities that support overall respiratory and immune balance.
For patients struggling with chronic sinusitis, allergic rhinitis, and related respiratory symptoms that complicate asthma control, specialized holistic care can make a meaningful difference.
SMUDRA Sinusitis Migraine Clinic offers focused Ayurvedic expertise in allergic rhinitis, chronic sinusitis, and related conditions that frequently overlap with difficult asthma.
